Grace Point Counseling Center offers confidential counseling services from licensed therapists and counselors that help people cope, resolve issues, restore hope and transform lives.

If you like, you can specify a counselor. Some of our counselors are in network providers for insurance; we can handle all out-of-network benefits. We can assist you with determining what your benefits are and with billing insurance. We will discuss your insurance with you prior to arriving for your first appointment. Forms
- Download our GPMFC informed consent form if you are seeking individual, couple, or marriage counseling. If you are a couple or married each person needs to complete this form.
- Download the Adult 18+ form along with the consent form if you are adult. Again, if you are a couple or married each person needs to complete this form.
- Download the Child/Adolescent form in addition to the informed consent form if you are bringing in a child/adolescent.
